We are now accepting messages for the 2012 Annual Report. Use the Annual Report Messages link in the menu on the left to view sample messages and download a pdf version of the application form. After completing the form, you can mail it to P.O. Box 90300, Washington, DC 20090-0300 with a check for your contribution. If you had a message in the 2011 Annual Report and would like to repeat the message without changes in the upcoming report, you can do so without submitting a new application form. Just go to our online donation site and choose the correct message category. Then, on the check-out page, answer "yes" to the question “Repeat 2011 Message?” There is a space to include the page number from the 2011 Annual Report, if you know it. You may include other identifying information (as it appears in the message) in the special instructions box. Supporters who would like to include a Gold Sponsor Message, a Silver Sponsor Message, or a Patron Listing in the 2012 Annual Report can also complete the entire transaction online. Go to our online donation site and choose the correct message category. Then, in the special instructions box on the check-out page, insert the appropriate length message (Gold = name, city and state plus a 50 character/space message; Silver = name, city and state plus a 25 character/space message; Patron = name, city and state only.)
